2. ASSUMPTION OF RISK
I understand that Moose Maine Kids programs may include activities such as:
Hunting (firearms, archery, crossbows)
Fishing
Boating and watercraft use
Hiking and wilderness travel
Remote lodging and camp environments
Use of ATVs, UTVs, trucks, boats, trailers, and other vehicles
Wildlife interaction
Outdoor weather and environmental exposure
I understand these activities involve inherent and unavoidable risks including serious bodily injury, permanent disability, illness, property damage, or death.
I voluntarily and knowingly assume all risks, both known and unknown, foreseeable and unforeseeable.
3. FIREARMS & ARCHERY ACKNOWLEDGMENT
I understand that certain programs involve supervised use of:
Rifles, shotguns, or other firearms
Ammunition
Bows and crossbows
Hunting-related equipment
I acknowledge firearms and archery equipment are inherently dangerous.
I understand my child may:
Handle firearms under supervision
Be in proximity to others handling firearms
Participate in live hunting scenarios
I certify that:
My child will follow all safety instructions.
I understand the risks associated with firearms and archery.
I voluntarily assume all risks related to such activities.
4. TRANSPORTATION & VEHICLE ACKNOWLEDGMENT
I understand that transportation may occur via:
I acknowledge that transportation carries inherent risks including collision, rollover, capsizing, and environmental hazards.
I grant permission for Moose Maine Kids representatives, volunteers, or guides to transport my child as necessary during program activities.
I assume all risks associated with such transportation.
5. OVERNIGHT & SUPERVISION ACKNOWLEDGMENT
I understand that some programs may involve overnight stays in lodges, cabins, camps, or remote locations.
I acknowledge:
Supervision will be provided by responsible adults, guides, or volunteers.
Outdoor environments may limit immediate access to medical facilities.
My child is expected to follow all rules and behavioral expectations.
6. RELEASE OF LIABILITY & HOLD HARMLESS AGREEMENT
In consideration of my child being permitted to participate, I hereby release, waive, discharge, and covenant not to sue:
Moose Maine Kids, Moose Maine LLC, its owners, officers, directors, employees, volunteers, guides, landowners, sponsors, affiliates, and partners
from any and all liability, claims, demands, losses, damages, or causes of action arising out of or related to participation, including claims arising from alleged negligence.
I further agree to indemnify and hold harmless the above parties from any claims brought on behalf of my child.
This release is intended to be as broad and inclusive as permitted under the laws of the State of Maine.
7. MEDICAL AUTHORIZATION
I certify that my child is physically able to participate.
In the event of illness or injury, I authorize Moose Maine Kids to obtain emergency medical treatment if I cannot be reached.
I accept full financial responsibility for any medical expenses incurred.

8. PHOTO, VIDEO & MEDIA RELEASE
I grant permission to Moose Maine Kids and Moose Maine LLC to photograph, video record, and otherwise capture my child’s image, voice, and likeness during activities.
I authorize use for:
Social media
Website
Promotional materials
Fundraising
Print publications
Press releases
Sponsor recognition
I understand:
No compensation will be provided.
All media becomes property of Moose Maine Kids / Moose Maine LLC.
Consent remains valid unless revoked in writing.
Revocation does not apply to already published materials.

9. MAINE GOVERNING LAW & VENUE
This agreement shall be governed by and interpreted in accordance with the laws of the State of Maine.
Any legal action arising from participation shall be brought exclusively in a court of competent jurisdiction located within the State of Maine.
10. SEVERABILITY
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
